Filling a Form

Learn How To fill out a form in Equip to collect information, document progress, and complete required paperwork as a Participant, Supporter, or Admin.

Forms in Equip are used to collect information, document progress, and complete required paperwork. Depending on how the form was created, you may be filling out either a PDF form or a custom form built directly in Equip.


📍 Access the Form

Forms may be available in one of the following places:

  • Forms can be accessed in several ways, depending on your role and permissions:

    • From the Home page → Fill a Form quick link

    • From a Participant’s Profile → Forms section (for supporters completing forms for a participant)

    • From the Forms Library (if an Admin has granted you access)

    • Through a direct link shared with you


📄 Open the Form

  • Click the name of the form you need to complete

  • Review any instructions or context provided at the top of the form

  • Confirm the correct participant is selected, if the form is participant-specific


✍️ Complete the Form

  • Fill out each question as accurately as possible

  • Required fields must be completed before submission

  • Forms may include:

    • Short or long text responses

    • Multiple choice or checkbox questions

    • Dates, ratings, or acknowledgements


📎 Filling Out a PDF Form

PDF forms are uploaded and managed by the Equip team and are used when a document must remain in its original format.

  • The form opens as a PDF file

  • Type directly into fillable fields, or

  • PDFs may require a signature or manual completion of certain fields

  • Once submitted:

    • The completed PDF is stored as a document

    • Responses are not broken out by question for tracking or reporting

PDF forms are best for externally required paperwork or forms that must remain in their original format.


🧩 Filling Out a Custom Form

Custom forms are built directly in Equip and are completed entirely in-app.

  • Questions appear as interactive fields within Equip

  • Required fields are enforced automatically

  • Responses are entered directly into Equip

  • After submission:

    • Data can be reviewed, approved, and tracked over time

Custom forms are ideal for data collection, progress tracking, and program-specific workflows.


Submit the Form

  • Review your responses before submitting

  • Click Submit to complete the form

  • After submission:

    • The form may be sent to an approver, if one is assigned
