Assigning a Subject on a Form Response

Some forms don’t require a subject at the time of submission—especially those sent to external users. Equip makes it easy to assign a subject later, so responses are tied to the right participant while still protecting privacy during collection.


🧾 Assign a Subject to a Form Response

  • Go to DocumentsForms from the left-hand menu

  • Click the name of the form

  • Find the response you want to update and click it

  • Click the three dots in the top right corner

  • Select Assign Subject

  • Choose the correct participant and confirm


🔐 Why This Matters

  • Respect privacy when collecting sensitive or anonymous info externally

  • Maintain accurate records by linking responses to participants after submission

  • Easily filter and review data across forms, responses, and reports


Equip Tip: If you’re sending external forms (like surveys, intake forms, or evaluations), consider leaving the subject field optional. Once reviewed, you can assign the subject internally—keeping things secure and streamlined.
