Adding a Form Signature

Learn how to add a signature when completing a form in Equip. This guide explains how to type or draw a digital signature and submit a signed form response.

Some forms in Equip require a signature to confirm acknowledgment, consent, or accountability. When a signature is required, a signature box appears at the bottom of the form and must be completed before the form can be submitted.


βœ… Complete the Form

  • Fill in each question on the form

  • Double-check all required fields before continuing


✍️ Add Signature

If the form requires a signature, you’ll find the signature box at the bottom of the page.

Type Signature

  • Click Type Signature

  • Type your name into the field

  • Your name will appear in cursive as you type

Draw Signature

  • Click Draw Signature

  • Use your mouse (or trackpad) to draw your signature in the signature box


πŸš€ Submit Response

  • Toggle Submit Another Response if you would like to complete the same form again

  • Select Submit Response

Your signed form will now appear in the Forms section of Equip.


πŸ“ Equip Tip: Once a form is signed and submitted, it cannot be edited. Be sure to carefully review all responses before adding your signature.
