Deleting Discharge Reasons

Learn how to delete Discharge Reasons in Equip and understand why reasons already used for participants cannot be removed to protect reporting and historical data.

Deleting a Discharge Reason allows your organization to remove options that are no longer relevant or were created in error. To protect historical data and reporting accuracy, discharge reasons that have been used for any participant cannot be deleted.


βš™οΈ Open Account Settings

  • Click your profile photo in the top-right corner of any page

  • Select Account Settings from the dropdown menu


πŸ“‹ Go to Discharge Reason Settings

  • In the left-hand menu, locate the Data Properties section

  • Click Discharge Reasons

  • This opens your Discharge Reason Library


πŸ—‘οΈ Delete a Discharge Reason

  • Locate the discharge reason you want to remove

  • If the discharge reason has not been used, click the context menu (three dots) on the right-hand side

  • Select Delete Discharge Reason

    • Note: If a discharge reason is associated with one or more participants, the Delete option will not appear. This prevents accidental removal of data tied to historical records and reports.


⚠️ Confirm Deletion

  • Review the confirmation message

  • Click Delete Discharge Reason to permanently delete the discharge reason


πŸ“ Equip Tip: If a discharge reason has already been used but needs clarification, consider editing the name instead of deleting it. This keeps past participant data intact while improving clarity moving forward.
