Equip's AI-powered import turns existing forms and assessments into structured Equip Forms or Assessments without rebuilding each question manually. Upload a supported file or paste a supported survey URL, and Equip creates a new item with extracted sections, questions, answer choices, and required fields for your team to review before use.


📄 How It Works

  1. Open Forms to import a form, or open Assessments to import an assessment.

  2. Click Import Form or Import Assessment at the top of the library.

  3. Choose an import method:

    • Upload a File — Drag and drop or browse to upload a PDF, Word (.docx), Excel (.xlsx), CSV, RTF, or plain text (.txt) file.

    • Import from URL — Paste a public Google Forms or Qualtrics survey link.

  4. Click Import.

  5. Equip processes the import in the background and sends a notification when the new form or assessment is ready.

  6. Open the imported item and review the questions, sections, answer options, and required fields before sharing or assigning it.

⚙️ Supported File Types & Sources

  • File uploads: PDF, Word (.docx), Excel (.xlsx), CSV, RTF, and plain text (.txt)

  • Maximum file size: 10 MB

  • URL imports: Public Google Forms links, including docs.google.com/forms and forms.gle links, and Qualtrics survey links

  • Microsoft Forms: Microsoft Forms links cannot be imported directly. Open the Microsoft Form in your browser, print or save it as a PDF, then upload the PDF file.

  • Use clean source files. Documents with clear question numbering, labeled sections, and defined answer choices import most accurately.

  • Use one form or assessment per file. Separate documents are easier for Equip to parse cleanly.

  • Prefer text-based PDFs. PDFs created from documents usually import more reliably than scanned images.

  • Check required questions. Review which fields were marked required before assigning the imported item.

  • Review before sharing. AI handles the first pass, but your team should confirm question types, answer options, and section breaks before using the form or assessment with participants.

Who can use this feature?
Users who have permission to create forms or assessments can use the import feature when it is enabled for their account.

Does importing overwrite an existing form or assessment?
No. Each import creates a new form or assessment, so existing items are not overwritten.

How long does an import take?
Most imports finish quickly, but processing happens in the background. Equip sends an in-app notification when the import completes or if it fails.

Can I import private Google Forms?
No. Google Forms must be publicly accessible enough for Equip to read the form content. If the form is private or restricted, export or print it as a PDF and upload the file instead.

What if the imported item does not look right?
Edit it like any other Equip form or assessment. You can update question text, reorder sections, adjust question types, and change answer options before using it.

What should I do if an import fails?
Check that the file type is supported, the file is under 10 MB, or the URL is accessible. If the source still will not import, try saving it as a PDF and uploading the PDF instead.
