📁 Go to Assessments
From the left-hand menu, click Assessments.
You’ll see a list of all your current assessments.
📄 Select Create Assessment
Click Create Assessment in the top right corner of the page.
This will take you to a page titled New Assessment.
+ Add Assessment Details
Status: Choose Active or Inactive.
Type: Select Shared (used across your organization) or Personal (specific to one individual).
Description: Add a short explanation of the purpose of the assessment.
Category: Assign the assessment to an existing category, or leave blank and decide later.
⚙️ Toggle Preferences
Allow Attachments – Let users upload files.
Require Signature – Collect a digital signature.
Include Summary Questions – Add overall reflection questions.
Show Response to Students – Share results with Participants.
If enabled, you can also Show Comments to Students.
🔔 Response Notifications
Toggle this on to choose who will receive notifications when an assessment is submitted.
📎 Add Attachments
Upload any supporting documents for the assessment.
❓ Add New Questions
For each question:
Write the Question.
Add a Description (optional guidance).
Select a Question Type:
Date Answer – Pick a date.
Likert Scale – Displays a horizontal rating matrix. You can set a Group Label (e.g., “Satisfaction Rating”) to organize related items.
Long Answer – Open text box.
Multiple Choice – Select multiple answers.
Number Answer – Enter a number (toggle Higher is Better or Enable Scoring).
Short Answer – Small text box.
Single Choice – Choose one option.
Additional toggles:
Add Unobserved Option
Enable Scoring
Allow Comments
Allow Attachments
Required
Repeat to add as many questions as needed.
📑 Add New Sections
Enter a Section Title.
Add a short Description.
Toggle Include in Exports if you want this section visible in exported reports.
✅ Save Your Changes
Select Save to confirm.
Your new assessment will now appear in the Assessments list.
Equip Tip: Start simple—add just the required details and a few key questions. You can always edit and expand your assessment later as your organization’s needs grow.
