Understanding Assessment Settings

Learn how to use assessment settings in Equip, including scoring, unobserved options, attachments, sections, and export preferences.

Equip’s Assessments feature includes customizable settings to support the way your organization tracks growth, measures progress, and gathers meaningful data. These settings ensure your assessments work the way you need them to—whether you’re tracking skill development, noting observations, or collecting structured responses over time.

Below is a breakdown of the available settings for each assessment and question type.


📋 Assessment-Level Settings

These settings apply to the entire assessment and can be adjusted when you’re first creating the form or later through the Edit Assessment screen.

  • Allow Attachments – Let users upload files as part of their responses.

  • Require Signature – Assessors must sign before submitting.

  • Include Summary Question – Adds a final open-ended field to summarize the overall assessment.

  • Show Responses to Participants – Make responses visible to the individual being assessed (Participants = students, clients, or residents).

  • Show Comments to Participants – Allow individuals to view any comments added to the form.

  • Enable Notifications – Notify team members when an assessment is submitted or updated.

  • Attachments – Attach up to 5 files to be shown to anyone completing the assessment.


❓ Question-Level Settings

When adding questions, you’ll have access to specific settings based on the question type.

Available for All Questions:

  • Allow Comments – Add a comment field to encourage additional notes.

  • Allow Attachments – Enable file uploads per question.

  • Required – Ensure this question must be completed before the form can be submitted.


🔘 Special Settings by Question Type

Some options appear only for specific question formats. Here’s what you can expect:

Single Choice Questions (only one answer selected):

  • Add Unobserved Option – Allows assessors to select “Unobserved” when they can’t evaluate a skill or behavior at the time. Useful for clarity and tracking skipped observations. Does not count against or in favor of score if Enable Scoring is toggled.

  • Enable Scoring – Assigns point values to each answer option to generate a total score. This helps quantify progress over time.

Number Answer Questions (numeric entry):

  • Enable Scoring – Treat number entries as part of a scoring system (e.g., for tests or progress ratings).

  • Higher is Better – Indicate whether higher values represent better performance (used for scoring logic).


📑 Adding Sections to Assessments

Sections allow you to organize your assessment into clear, labeled parts—ideal for longer assessments or when grouping related questions (e.g., Communication Skills, Daily Living, Job Readiness).

When you add a section, you can:

  • Title the Section – This title will appear in the assessment to help guide the user and structure the form.

  • Description (optional) – Add a short explanation or instructions that apply to the section. This is helpful for providing context before a group of related questions.

  • Include in Exports – Toggle this option to decide if the section title and description should appear in exported versions.

Use sections to improve readability, clarify expectations, and enhance how data is presented externally.


🛠 Editing Settings Later

  • You can adjust assessment and question settings at any time by clicking Edit on an existing assessment.

  • Updated settings will apply immediately and reflect in all future submissions.


Equip Tip: Use the Unobserved option to avoid guessing or skipping entries—especially helpful when evaluating something, like behaviors or classes, that may not occur during every session.
