Equip can now automatically convert your existing forms and assessments into fully structured Equip forms — no manual rebuilding required. Upload a file or paste a URL, and our AI will parse the content, detect questions, sections, and answer options, and create a ready-to-use form in seconds.
📄 How It Works
Navigate to Forms or Assessments in Equip.
Click the Import button at the top of the list.
Choose one of two import methods:
Upload a File — Drag and drop or browse to upload a document. Supported formats: PDF, Word (.docx), Excel (.xlsx), CSV, RTF, and plain text (.txt). Maximum file size is 10 MB.
Import from URL — Paste a link to a Google Forms or Qualtrics survey.
Click Import.
The import processes in the background. You'll receive a notification when your form is ready — or if something went wrong.
Review the imported form. Questions, sections, and answer options are automatically mapped to Equip's form structure.

💡 Best Practices
Use clean, well-structured source documents. Forms with clear question numbering, labeled sections, and defined answer choices import most accurately.
Review after import. AI does the heavy lifting, but always review the imported form to confirm question types and answer options mapped correctly.
Prefer digital formats over scanned PDFs. Text-based PDFs import more reliably than scanned images.
One form per file. Upload a single form per import for the best results.

Who can use this feature?
Any user with permission to create forms or assessments can use the import feature.
How long does the import take?
Most imports complete within 30 seconds. You'll receive an in-app notification when it's done.
What if the import doesn't look right?
You can edit the imported form just like any other form in Equip. Adjust question types, reorder sections, or update answer options as needed.
Can I import the same form more than once?
Yes. Each import creates a new form, so you won't overwrite existing work.
Does this work for assessments too?
Yes — the same import feature is available on both the Forms and Assessments pages.
What happens if the import fails?
You'll receive a notification letting you know the import couldn't be completed. Common causes include unsupported file formats, files that are too large, or URLs that can't be reached. Try again with a different file format or check the URL.
