Assigning Forms to Pathway Steps

Learn how to assign forms to pathway steps so they appear automatically during a participant's pathway progress.

Forms and Pathways are two powerful features in Equip β€” and when you connect them, you can ensure the right forms are completed at the right stage of a participant's journey. Assigning forms to pathway steps means your team won't have to remember which forms go with which step. They'll appear automatically when a participant reaches that point in their pathway.


βš™οΈ How It Works

When you assign a form to one or more pathway steps, that form becomes linked to those steps. When a participant's pathway progress reaches an active step, the assigned form(s) appear as clickable icons on the participant's Pathway Progress widget. Staff can click the icon to open and fill out the form directly from the pathway β€” with the participant already set as the subject.

This connection works in both directions:

  • From the Form side: you choose which pathway steps the form should be assigned to.

  • From the Participant's Pathway Progress: assigned forms appear as icons next to active steps, ready to be completed.


πŸ“„ How to Assign a Form to Pathway Steps

  1. Navigate to Forms from the main menu.

  2. Click on the form you want to assign to open its detail page.

  3. Click the context menu (three-dot icon) and select Edit Form.

  4. Scroll down to the Assign to Pathway Steps toggle and turn it on.

  5. A Pathway Steps dropdown will appear. Select one or more steps from the list. Steps are organized by pathway name (e.g., Intake - Initial Assessment).

  6. Save the form.

That's it β€” the form is now linked to those pathway steps.


πŸ“‹ Completing Assigned Forms from a Pathway

  1. Open a participant's profile and navigate to their Pathways section.

  2. On the Pathway Progress widget, look for the form icon(s) next to any active step.

  3. Click the icon to open the form. The participant is automatically set as the subject.

  4. Complete and submit the form as usual.

Note: Form icons only appear on steps that have a status other than Not Started. Once a step is in progress, assigned forms become available.

  • Less manual work: No need to navigate away from the pathway to find and fill out forms β€” the participant is already linked as the subject.

  • Clear visibility: The form detail page shows which pathway steps it's assigned to under Assigned to Pathway Steps in the sidebar.

  • Name your forms clearly so staff can easily identify them from the pathway step icon tooltip.

  • Assign forms during setup β€” configure assignments when building your pathways and forms, so everything is ready before participants are enrolled.

  • Review assignments periodically β€” if your pathways or forms change, check that the right forms are still linked to the right steps.

  • Use pathway step codes to keep your steps organized, especially when assigning forms across multiple pathways.


❓ FAQs

Can I assign the same form to multiple pathway steps?
Yes. When you turn on Assign to Pathway Steps, you can select as many steps as you need from the dropdown.

Can a pathway step have more than one form assigned?
Yes. Each assigned form will appear as a separate icon on the step in the participant's Pathway Progress widget.

Where do I see which steps a form is assigned to?
On the form's detail page, look in the right sidebar under Assigned to Pathway Steps. It lists each linked pathway and step.

Why don't I see the form icon on a pathway step?
Form icons only appear on steps with a status other than Not Started. The participant's step progress must be active for assigned forms to show.

What happens if I remove a form's pathway step assignment?
The form will no longer appear on that step in the Pathway Progress widget. Existing responses already submitted through that step are not affected.

Do I need the Pathways feature enabled to use this?
Yes. The Assign to Pathway Steps toggle only appears on the form editor when the Pathways feature is enabled for your account.
