Assigning forms to pathway steps helps team members complete the right form at the right point in a pathway. This supports CRP services, Higher Ed student support workflows, and other programs where forms should be tied to a specific step in the process.


⚙️ How It Works

When you assign a form to one or more pathway steps, Equip connects that form to those steps. When a participant or student is actively working through the step, team members can open the assigned form from the pathway workflow with the participant already selected as the subject.

This connection works in both directions: the form setup shows which pathway steps it is assigned to, and the participant’s pathway progress shows assigned forms when they are available for that step.


📄 How to Assign a Form to Pathway Steps

  1. Open Forms.

  2. Select the form you want to assign.

  3. Open the context menu and select Edit Form.

  4. Turn on Assign to Pathway Steps.

  5. Use the pathway step picker to select one or more steps.

  6. Save the form.


📋 Completing Assigned Forms from a Pathway

  1. Open the participant’s profile.

  2. Open Pathways.

  3. Find the active pathway step.

  4. Select the form icon for the assigned form.

  5. Complete and submit the form.

Form icons appear when the participant’s step progress is active. A step that has not started yet does not show its assigned form icons.

  • Name forms clearly so team members can recognize them from the pathway step.

  • Assign forms while setting up pathways so they are ready before participants or students are enrolled.

  • Review assignments when forms or pathways change.

  • Use pathway step codes when your organization needs consistent service, course, billing, or reporting references.


❓ FAQs

Can I assign the same form to multiple pathway steps?
Yes. Turn on Assign to Pathway Steps and select as many steps as needed.

Can a pathway step have more than one form assigned?
Yes. Each assigned form appears separately when that step is active.

Why do I not see the form icon on a pathway step?
The assigned form appears when the step has started. It does not appear on steps that are still Not Started.

What happens if I remove a form’s pathway step assignment?
The form no longer appears from that pathway step. Existing form responses are not removed.

Do I need Pathways enabled to use this?
Yes. The pathway step assignment option is available when Pathways are enabled for your account.
