Creating a goal in Equip allows your team to define meaningful outcomes and track progress through clear, measurable steps. Each goal is broken down into Waypoints, making it easier to monitor development, update supports, and celebrate achievements. With consistent use, Goals in Equip help teams stay aligned and focused on person-centered growth.
š Start a New Goal
Go to the individualās Profile
Click Goals ā Add Goal
This opens the goal builder where youāll enter the key information.
š§ Add a Goal Title
This is the headline for the goal. It should be short, specific, and action-focused.
Example: āIncrease independence with transportationā or āBuild confidence in workplace communication.ā
š Write a Description (Optional)
Use the description to explain the purpose behind the goal. Why is it important? What does success look like? What kind of support will be provided?
Example: āThis goal focuses on helping Jordan take the campus shuttle independently to and from class.ā
š Set a Start and End Date
Start Date: When youāll begin tracking progress
End Date: When you plan to review or complete the goal
Timelines keep goals focused and ensure consistent follow-up. Dates can be updated as needed.
š¦ Choose a Status
Set the goal status to reflect where things stand: Active, Completed, or Paused.
Most goals will begin as Active.
š§ Add Waypoints
Waypoints are the milestones that lead toward completing the goal. For each waypoint, enter:
Title ā a short description of the step (e.g., āRides shuttle with supportā)
Description (optional) ā define what success looks like
Target Date ā expected completion date
Baseline and Target ā use for measurable progress (optional)
Status ā mark as Not Started, In Progress, or Completed
You can add multiple waypoints and rearrange them as needed.
š¾ Save and Monitor Progress
After saving, the goal appears on the individualās Goals list.
You can return at any time to update Waypoints, add Progress Updates, or make adjustments based on new information or support needs.
