Setting and tracking goals is a powerful way to support personal growth, build independence, and celebrate meaningful progress. In Equip, Goals allow you to define what success looks like for an individual, then break that goal into smaller, achievable steps called Waypoints. As progress is made, you can add Progress Updates that capture observations, notes, and reflections over time. This structure helps teams stay aligned, track changes clearly, and adjust supports based on what’s working. Whether the focus is life skills, employment readiness, or personal development, Equip makes goal tracking collaborative, flexible, and easy to manage.


Prerequisites

Before using the Goals feature it must be enabled on your account. Contact the Equip team if you'd like to utilize Goals and aren't seeing them on user profiles.


Getting Started with Goals

Understand the Structure of a Goal

  • Each Goal is made up of one or more Waypoints.

  • Waypoints are small, trackable steps toward a larger objective.

  • This breakdown makes complex goals more manageable.

Add Progress Updates

  • Use Progress Updates to log observations, reflections, and team notes.

  • Updates can only be added to Waypoints that are In Progress.

  • Once a Waypoint is marked Completed, it remains visible but can no longer be edited.

Track and Adjust Over Time

  • Regularly review Goals and Waypoints to monitor what’s working.

  • Use insights from Progress Updates to make timely adjustments and celebrate wins.

The Goals feature can be used in a variety of ways, including:

  • Align Goals with person-centered plans to track meaningful progress.

  • Keep Waypoints small and specific to make progress measurable.

  • Encourage team members to add regular Progress Updates for consistency.

  • Revisit goals often during meetings, check-ins, or reviews.

  • Use Equip’s flexible structure to support a variety of goals—from life skills to employment readiness.
