Form Expirations Report

Learn how to run the Form Expirations Report and understand its filters and exported fields.

The Form Expirations Report exports active form expiration records into a spreadsheet. It is useful for identifying forms that are due to expire, reviewing who they are assigned to, and sharing direct completion links.


⚙️ How it works

  1. Go to Reporting.

  2. Open the Forms tab.

  3. In the Report dropdown, select Form Expirations.

  4. Optionally choose Accounts, Expiration Date Range, Forms, Categories, Tags, Users, and Groups.

  5. Click Generate to export the report.

⚙️ Filters and export fields

  • Accounts is available for umbrella accounts.

  • Expiration Date Range filters active expirations by expiration date.

  • Forms limits the report to selected forms.

  • Categories limits the report to selected categories.

  • Tags limits the report to tagged forms.

  • Users limits the report to selected users.

  • Groups limits the report to selected groups.

This report exports the following fields:

  • Form Title

  • User Name

  • Expiration Date

  • Team Link

  • Non-Team Link

Only active expiration records are included. The report is limited to custom forms.


💡 Best practices

  • Use a date range when reviewing upcoming expirations for follow-up work.

  • Use user and group filters when preparing outreach for a specific set of people.

  • Use the team and non-team links based on how the recipient will complete the form.

  • Review this report regularly if your program depends on recurring form completion.


❓ FAQs

What does the Expiration Date Range filter look at?
It filters active expirations by expiration date.

What is the difference between Team Link and Non-Team Link?
Team Link points to the internal form response path, while Non-Team Link points to the linked external form path.

Will inactive or past expirations appear?
No. This report only includes active expiration records.

Why is a user name blank on some rows?
A row can have no user name if the expiration is not tied to a specific user.

Does this report include all form types?
No. It is limited to custom forms.
