What is the Form Expirations Report?

The Form Expirations Report helps administrators track forms with time-bound data collection rules that require periodic updates. By identifying upcoming expirations, this report ensures that necessary forms are resubmitted on time, maintaining compliance and accuracy in record-keeping.


πŸ“ Where to Find It:

  • Navigate to Reporting in the Equip menu.

  • Select Forms from the available options in the list under Reporting.

  • Choose Form Expirations to generate the report.


Report Structure & Columns

The Form Expirations Report includes the following information in separate columns within the exported file:

  • Form Title – Name of the expiring form

  • User Name – Name of the user assigned to the form

  • Expiration Date – The date by which a new response is required

  • Team Link – A shareable link for team members with web access

  • Non-Team Link – A shareable link for users without web access or non-Equip users who need to complete the form


Filtering the Report

Before exporting the report, you can refine the data using optional dropdown menus:

  • Expiration Date Range – Select a date range to check for expiring forms. (Filters forms based on their expiration dates.)

  • Categories – Choose a form category (e.g., Academic) to check for expiring forms within that category. (If left blank, all categories are included.)

  • Users – Select specific users to check for expiring forms. (If left blank, all users are included.)

  • Groups – Select user groups to check for expiring forms. (If left blank, all groups are included.)

Once selections are made, click Generate Report to export the data.
