Duplicating a form in Equip creates a copy of an existing custom form so your team can reuse its structure without rebuilding sections, questions, and answer options from scratch. This is helpful when you need a similar form for a new program, workflow, or reporting process.
đ How It Works
The Duplicate Form option is available from the form detail page. When a form is duplicated, Equip creates a new copy titled with (Copy) and opens it in edit mode so you can make changes before using it.
đ Duplicate a Form
Go to Forms from the left-hand menu.
Open the form you want to copy by clicking the form name.
On the form detail page, click the three-dot menu in the top-right corner.
Select Duplicate Form.
Equip will open the copied form in edit mode. Update the title, questions, settings, or permissions as needed.
Click Update Form to save your changes.

Custom forms only - The duplicate option is available for custom forms.
Create permission required - Your role must allow you to create forms.
Not available while editing - If you are on the Edit Form page, the menu may only show Delete Form or Discard Form. Go back to the form detail page to duplicate the form.
Shared umbrella forms - Forms shared from an umbrella account may not show the duplicate option.

Does duplicating a form copy previous responses?
No. The copied form starts with no responses.
Why do I only see Delete Form or Discard Form?
You may be on the Edit Form page. The Duplicate Form option appears on the form detail page. If it still does not appear there, the form may not be a custom form, may be shared from an umbrella account, or your role may not have permission to create forms.
Can I edit the copied form before using it?
Yes. After you select Duplicate Form, Equip opens the copied form in edit mode so you can update it before collecting responses.
Will the original form change when I edit the copy?
No. The copied form is separate from the original form.
