Pathway Centralization brings pathway progress, step status, linked records, and closure workflows into one place. It helps team members review the full context around a participant’s pathway instead of switching between separate pages of Equip.
📄 How It Works
When Pathway Centralization is enabled for your account, pathway progress opens into a detailed page for the selected participant and pathway. This page shows the pathway overview, step timeline, custom field values, and linked activity such as goals, forms, assessments, notes, and documents when the user has permission to view those modules.
Open a participant's profile.
Go to Pathways.
Select a pathway progress record.
Review the pathway details, step timeline, and linked activity panels.
Use Update Progress to edit dates, included steps, supporters, authorizations, or custom field values when you have permission.
🔗 Linking Records to Pathways
Supported records can include a Pathways field with the placeholder Link to Pathways or Steps. Use this field to connect the record to a participant’s pathway enrollment or a specific pathway step.
Goals — link goals to the pathway or step they support.
Forms — link completed form responses to the relevant pathway context.
Assessments — link assessment responses to the pathway or step they relate to.
Notes — link notes to the pathway or step they document.
Documents — link uploaded documents to the pathway or step they support.
The linked activity page only shows records the viewer is allowed to access. Module permissions still apply.
🧭 Progress Status and Closure
Step statuses — steps can show Not Started, In Progress, Completed, or Discharged depending on dates and discharge activity.
Discharge — authorized users can discharge started pathway services, including discharge date, reason, and notes.
Close Progress — authorized users can close a pathway enrollment after in-progress steps are completed or discharged.
Reopen Progress — closed pathway progress can be reopened when the user has permission.
Closed Pathways Only — the team members Pathway Progresses list includes a filter for closed pathway progress when centralization is enabled.

💡 Best Practices
Link records to the most specific place that makes sense. Use a step link when the record belongs to a specific step, and a pathway link when it applies to the whole enrollment.
Keep step start and end dates current so status filters and progress views stay accurate.
Use discharge when a started service stops before normal completion.
Close pathway progress when the overall enrollment is finished and no steps are still in progress.
Review permissions if a user cannot see linked activity they expected to see.

Does Pathway Centralization replace the Pathways feature?
No. It adds a centralized detail view and linking workflows on top of Pathways.
Why do I not see a linked activity section?
The section appears only when your account has centralization enabled and you have permission to view that module.
Can I link a record to a pathway step instead of the whole pathway?
Yes. When available, choose the pathway step option if the record belongs to a specific step.
Can I close pathway progress while a step is still in progress?
No. Complete or discharge in-progress steps before closing the pathway progress.
Do permissions still apply to linked records?
Yes. Linked activity respects the viewer’s module and participant access.
