Pathway Centralization adds a View Details page to every pathway progress on a participant's Pathways tab. That page shows the round's status and completion, the configured dates, the step timeline, the goals, notes, forms, assessments and documents linked to the pathway or its steps, and the authorization's invoices, and it is where Discharge Step, Close Progress and Reopen Progress live. It also adds a Pathways field on notes, goals, forms, assessments and documents to link them to a pathway or step, discharge and closed filters on the Progresses list, and discharge and closure columns on the Pathway Progress report. Nothing about existing pathways, steps, dates, authorizations or billing changes.
Participants are labelled Clients, Students, Residents or Explorers depending on the account type. This article calls them participants.
Who can use it, and where
Web app only. The mobile app shows step dates and statuses but none of the linked activity, discharges or closures.
View Details needs the Pathways module at Viewer or higher and the participant in your scope of support. Discharge Step, Close Progress and Reopen Progress need Supporter or higher.
Each linked activity section only shows when your role can see that module on participant profiles, for example the Notes module for linked notes.
Pathway Centralization is on for every account type except family accounts and is not available to umbrella accounts. It is turned on for your account by the Equip team. If you do not see View Details on a pathway progress, ask us to enable it.
Steps to open a pathway progress
Open the participant's profile and choose Pathways in the left menu.
Click View Details on the pathway, or click the pathway's name.
Read the header: the round's status badge, "N of M steps completed" with a discharged count, the percentage complete, a Current Step tile and one tile per configured date, and any Pathway Data Fields.
Read the Steps timeline: each included step with its badge, date range, days elapsed, supporter, discharge details and step data fields, plus a Document button for forms assigned to a started step.
On the right, expand Invoices and the linked activity sections: Goals, Forms, Assessments, Notes and Documents. Each row opens the record and shows whether it is linked to the pathway or to a specific step.
Use Update Progress in the header to change dates, steps, supporters, authorizations and data fields, and the menu next to it for Close Progress or Reopen Progress.
Steps to link a record to a pathway or step
Open the note, goal, form response, assessment response or document, or create a new one for the participant.
In the Pathways field, placeholder Link to Pathways or Steps, pick the pathway round or one of its steps. Each option shows the round's status, so two rounds of the same pathway can be told apart.
Save. The record now appears under the matching section on View Details, and its Pathway and Pathway Step show in the detail reports that carry pathway columns.
Link to the step when the record belongs to one phase of service and to the pathway when it applies to the whole round. Events and routines cannot be linked from the picker at the moment.
Closing and reopening a pathway progress
Close Progress marks the round finished. It is in the menu next to Update Progress on View Details and is refused while any step is In Progress: "All in-progress steps must be completed or discharged before closing this pathway progress." The menu item is greyed out with "Complete or discharge the in-progress steps before closing." Not Started steps do not block it.
If the participant has a Case on that round and it is the case's last open round, the confirmation offers Also close Case ID .... Reopening the progress later does not reopen the case.
A closed round shows a Closed badge on the Pathways tab and in the View Details header with who closed it and when, and Pathway Closed next to each of its steps on Pathways, then Progresses. It no longer blocks starting the same pathway again.
Reopen Progress removes the closure. The confirmation reads "Reopen this pathway progress? The closure record will be removed and the enrollment becomes active again."
Closing never changes the participant's profile status. Use the discharge form's third option, or the profile itself, to make a participant inactive.
Limits and what the errors mean
Linked records are never deleted by pathway actions. Deleting a step or pathway from the library, or deleting a pathway progress, unlinks them; the deletion preview says "N linked records (unlinked, not deleted)" and, for a step, lets you tick which links to keep at the pathway level.
Deleting a pathway progress is still permanent for the round itself: dates, supporters, discharges, closure and recorded custom field values go with it. The confirmation reads "Are you sure you want to delete this pathway progress? It will be permanently removed and all notes associated with it will be disconnected. This action cannot be undone."
Pathways, then Progresses gains Discharged, Discharged (completed) and Discharged (incomplete) status options, a Discharge Reason filter, a Closed Pathways Only toggle, a Discharge batch action and a Discharged tile.
The Pathway Progress report gains enrollment status, closed date, discharge date, reason, disposition and notes columns and the Pathway Discharges report becomes available.
There is no Download Summary or print button on View Details today. Use the Pathway Progress report for a spreadsheet of a round.
FAQ
Does Pathway Centralization change my existing pathways?
No. Pathways, steps, rounds, dates, supporters, authorizations and invoices stay exactly as they were. It adds the View Details page, record linking, discharges, closing and reopening, and the matching filters and report columns.
Which records can I link to a pathway?
Goals, notes, form responses, assessment responses and documents, using the Pathways field with the placeholder Link to Pathways or Steps on each of those records. Events and routines are not in the picker at the moment.
Should I link a record to the pathway or to the step?
Link to the step when the record belongs to one phase of service, such as a job coaching note; link to the pathway when it applies to the whole round, such as an overall employment goal. Step links give more precise filtering and reporting.
Can I close a pathway while a step is still in progress?
No. Close Progress is greyed out with "Complete or discharge the in-progress steps before closing." Give each in-progress step an End Date, or discharge it, then close. Steps that never started do not block closing.
Does closing a pathway make the participant inactive?
No. Close Progress only marks that round finished, and the participant stays active. To deactivate the profile as well, use Discharge Step with "Discharge from this step, close pathway progress and deactivate the profile", or set them inactive from their profile.
Why don't I see a linked activity section, or View Details at all?
A section only shows when your role can see that module on participant profiles, so a role without Notes access sees no Notes section. If View Details itself is missing, Pathway Centralization is not on for your account, or your role's Pathways module is at None.
Can I undo a discharge or a closure?
Yes. Undo next to the discharge on the step timeline removes it and, if the discharge recorded the step's End Date, that date too. Reopen Progress removes a closure. Neither reopens a Case ID closed at the same time or reactivates a deactivated profile.
