Copy an existing form

How to duplicate a custom form in Equip, exactly what the copy carries over, and what you have to set again by hand.

Open the form, click the three-dot menu at the top right of the form's page, and choose Duplicate Form. Equip makes a copy named after the original with (Copy) on the end, and opens it straight away in edit mode so you can rename it before anyone uses it. The original is untouched and the copy starts with no responses.

Participants are labelled Clients, Students, Residents or Explorers depending on the account type. This article calls them participants.

Who can do this, and where

  • Duplicating happens in the Equip web app only.

  • You need the Forms module set to Administrator on your role, or you need to be an account owner. Roles are set under Settings, Permissions, Custom Roles.

  • Duplicate Form only appears on a form's own page, not on the rows in the Form Library and not while you are on the Edit Form page.

  • It only appears for custom forms, and never for a form shared with you by an umbrella account.

  • Forms is turned on for your account by the Equip team. If you do not see Forms in the left menu, ask us to enable it.

Steps

  1. Click Forms in the left navigation.

  2. Click the form's title to open its page. Do not use the three-dot menu on the list row, which has no duplicate option.

  3. Click the three-dot menu to the right of Fill Form.

  4. Choose Duplicate Form.

  5. Equip opens the copy in edit mode. Change the Title so staff can tell the two apart, and adjust anything else you need.

  6. Click Update Form.

Limits and what the errors mean

  • The copy is created with the same Status as the original. Duplicating an Active form gives you a second Active form immediately, so rename it before sharing any link.

  • The copy gets its own public link. Links and QR codes already handed out for the original keep pointing at the original.

  • If the form is a PDF document form, there is no Duplicate Form option. Copy the template again with Use Template instead.

  • If you only see Delete Form or Discard Form in the menu, you are on the Edit Form page. Go back to the form's page.

  • If the three-dot menu has no Duplicate Form even on the form's page, the form is a document form, it is shared from an umbrella account, or your role is not at the Administrator level for Forms.

  • Duplicating does not copy the form-level attachments. Re-upload them on the copy if responders need them.

FAQ

Does duplicating a form copy previous responses?
No. The copy starts with no responses and a response count of zero. The original keeps every response it had.

Why do I only see Delete Form or Discard Form?
You are on the Edit Form page, whose menu only offers deleting. Duplicate Form lives on the form's own page, in the menu next to Fill Form.

Will editing the copy change the original?
No. The two forms are separate records from the moment the copy is made. Editing questions, preferences or subscribers on one has no effect on the other.

Are the approvers and notification subscribers copied?
Yes. Response subscribers, approvers and expiration subscribers all carry over, as do the roles listed under Restrict Access. Check them before collecting responses, because a copy made for a different program often needs different people.

Why are the attached files missing from my copy?
Duplicating copies the form's structure and settings but not its uploaded attachments or its tags. The attachment instructions text comes across, so re-upload the files on the copy and the instructions will match again.

Can I duplicate a PDF document form?
No. The option is only offered for custom forms. To get another copy of a PDF form, use Use Template on the Form Library page and copy the template again.

Can I duplicate a form an umbrella account shared with us?
No. Forms shared from an umbrella account cannot be duplicated, edited or deleted in the sub-account. Ask the umbrella account to make the change or to share a second form.
