Request a form or assessment after an event

Turn on Request Form or Request Assessment on an event so registrants in chosen roles get a task and email five minutes after it ends, with one reminder a day later.

To collect a form or assessment after an event, open the event's Data Collection section, turn on Request Form or Request Assessment, choose the form or assessment and the roles that should complete it, and save. Five minutes after the event ends, every registrant in those roles gets a task and an email asking them to complete it, and the responses show up on the event page.

Participants are called Clients, Students, Residents or Explorers depending on your account type; this article says participant.


Who can do this, and where

  • Team members who can create or edit events on the web (Calendar level Supporter or Administrator). The settings are on the web event form only.

  • Recipients complete the form or assessment from the task in the mobile app or on the web, or from the link in the email.


Steps

  1. Open the event and select Edit Event, or start a new event.

  2. Expand Data Collection.

  3. Turn on Request Form or Request Assessment.

  4. Choose the Form or Assessment.

  5. Under Send to Roles, pick the roles whose registrants should complete it. By default every team role is selected; add the participant role if participants should fill it in themselves.

  6. Optionally choose a Notify User who is told when responses come in, on top of the form's or assessment's own subscribers.

  7. Select Add Event or Update Event.


What happens after the event

  • Five minutes after the event's end time, each registrant whose role is in Send to Roles gets a task "Complete [title]" due the day after the event, plus an email with a link.

  • If the task is still open one day later, one reminder email is sent. No more are sent after that.

  • Responses appear on the event page in a tab named after the form or assessment, alongside the Attendance tab. For an assessment, scores are charted over time.

  • From the registrant list, the three-dot menu next to a person offers Copy Form Link or Copy Assessment Link so you can hand them the link directly.

  • Each occurrence of a recurring event sends its own request after that occurrence ends.


Limits and gotchas

  • The request is only scheduled when at least one participant is registered for the event. An event with only team members registered sends nothing, even if Send to Roles lists team roles.

  • The request is scheduled when you save the event. If the event has already ended when you turn data collection on, nothing is sent; use the copy-link menu instead.

  • Turning Request Form or Request Assessment off removes the pending request and its tasks. Submitted responses are kept.

  • Deleting the event keeps submitted responses, but they are no longer shown on an event page.

  • For a form that should be completed before or during the event, put its link in the event Description instead. Use the participant-specific form link if the response must attach to that participant's profile.


FAQ

When is the form or assessment sent?
Five minutes after the event's end time, to every registrant whose role is selected under Send to Roles. It is a task in Equip plus an email; it is not sent at the start of the event.

Why did nobody receive the request?
Check that a participant is registered (requests need at least one), that the roles you want are under Send to Roles, and that the event had not already ended when you saved. Team-member-only events do not send requests.

People keep getting reminders. How do I stop them?
Each event occurrence sends at most two messages: the request five minutes after the end, and one reminder a day later if the task is still open. Completing the task stops it. For a recurring event, turn off Request Form on one occurrence with Update Future Events on to stop future occurrences.

Where do I see the answers?
On the event page, in the tab named after the form or assessment. Each answer links to the full response. Assessment scores are also charted per question.

Can I send the form during the event instead?
Not automatically. Paste the form link into the event description so registrants can open it from the event, or use Copy Form Link on the registrant list once the request exists.

Can I collect from team members and participants at once?
Yes. Select both kinds of roles under Send to Roles. Each registrant gets their own task; a team member's assessment task names the participants who attended.
